As men age, it is common to develop difficulty achieving and maintaining an erection – this is known as erectile dysfunction. By the age of 40, more than one in five men will be diagnosed with erectile dysfunction. By the age of 70, more than half of men will have erectile dysfunction. Men with high blood pressure, diabetes, and/or obesity are at increased risk of erectile dysfunction. Men with erectile dysfunction should know that their risk of coronary artery disease is increased compared to the general population, as the underlying cause of both conditions is similar.
Initial evaluation for erectile dysfunction involves identifying underlying risk factors, and sometimes, laboratory testing to ensure there is not a hormonal component.
Initial treatment for erectile dysfunction generally consists of lifestyle modification – changes to the diet and increased exercise. In patients where this is not an option, we will generally start with oral medication, which can be taken regularly or as needed.
Vacuum Erection Device (VED) is an appliance used by men with erectile dysfunction (ED) to achieve an erection. It is generally used in men who have failed or are unable to use oral medication options for ED. The device is placed over the penis, and a vacuum is created, pulling blood into the erectile bodies. A constrictive ring is placed at the base of the penis to hold the blood in. When the erection is no longer desired, the ring is removed. At Urology of Middle TN, we have professionals available to counsel patients on how to most effectively use these devices.
Intracavernosal therapy for ED – A small needle is inserted into the corpora cavernosa (erectile body of the penis) and the medication is injected. Patients are instructed in our office on how to do this safely and how to adjust the dosing for optimal performance. The vast majority of men have minimal discomfort with the injection process and the effects of intracavernosal medication for ED are quite robust.
Inflatable Penile Prosthesis – An implantable device that is used in men who failed all other non-surgical options for erectile dysfunction. During the procedure, inflatable cylinders are implanted into the erectile bodies. The cylinders are attached to a pump, which is placed in the scrotum and a reservoir of fluid placed in the pelvis. When an erection is desired, the pump is activated and fluid fills the cylinders creating an erection. Penile sensation and the ability and sensation of climax is preserved.
The procedure is performed under general anesthesia at either a surgery center or hospital. A small incision is made either above or below the penis and the all of the device components are implanted through this incision. Most patients are able to go home the same day. Healing takes approximately 6 weeks, after which the device can be used.
