Appointment Scheduling
  • Please email us at office@clarksvilleurology.com  or give our friendly staff a call at 931-291-9150 to schedule your appointment.
  • As a courtesy, we will make every effort to contact you before your appointment to remind you of your visit. This is done as a courtesy. It is your responsibility to remember and keep your appointments once they are made.
What to Bring to Your Appointment
  • Your insurance card and photo identification card. Due to risk of identity theft and for your protection, you will not be seen without photo identification.
  • Copayment / payment for your visit. Your insurance may require us to collect a copay at the time of your visit. You may be rescheduled if payment is not arranged prior to your appointment.
  • Physician referral forms, if required by insurance.
  • List of current prescription and any over-the-counter medications you are taking, including how much (dose), and how often (frequency) you take them.
  • Any recent imaging studies (x-rays, CT, ultrasounds, bone scans, etc.) in film or electronic (CD, DVD, DICOM, JPG, etc.) format. To avoid any delays in your treatment, it is imperative that YOU bring these imaging studies to your office visit.
  • Any recent lab / blood / urine test results.
  • Any past Urology records from your previous urologist
  • Completed Registration Forms. Completing forms and returning them to our office before arriving for your appointment will save you time on the day of your visit. Click on the forms below to download.

Phone Calls and Inquiries
  • If your questions can not be adequately answered by one of our medical staff, we will forward your concerns to our providers, who will then respond to your question. We will usually address your concerns the same business day.
Prescriptions
  • Prescriptions and refills will be issued only during business hours when the office is open.
  • We utilize electronic prescribing technology, which means your prescriptions are directly entered into the pharmacy’s computer from Clarksville Urology. No more waiting for a fax to “go through”, no more transcription errors, no more losing prescriptions, and no more waiting forever at the pharmacy. Please inform us of the specific pharmacy, its location, and phone number for prompt service.
  • Please call at least 24-hours before a refill is needed to ensure continuity of medication.
  • Your medication refill requests also arrive at our office electronically. Refill requests are efficiently authorized through our electronic medical records system. This means you won’t have to wait for days for your refill prescription requests to arrive at the pharmacy.
  • For those who may have difficulties paying for their prescriptions, search the Internet for “patient assistance programs”. Below is a short list of programs for patients in need:

Lab and Imaging Results
  • All lab and imaging reports are published to the patient portal as soon as they are reviewed by your provider. 
  • Do NOT assume that a lab results is “normal” or “negative” merely because you have not heard from us. You should assume an “abnormal” or “positive” lab result and you should call us to let us know that you have not received your results. On rare occasions, generally due to issues with electronic transmission of reports, we do not receive the results of a test even though you had it done.
  • You should hear from our office within two (2) weeks (14 days) of getting any test done. In almost every scenario, you will be scheduled to follow up with an office visit to discuss any lab or imaging findings and its implication on your health.
Patient Data Security

We take the security of your personal and medical information extremely seriously. Maintaining your medical records electronically means your information can not be easily stolen as is the case with paper charts. Your information is further protected by our employees – all of whom have undergone background checks.

We have a secure Patient Portal which can be accessed at https://health.healow.com/ouradvancedhealth.

The patient portal is the most reliable way to contact us for non-emergency issues/questions.
